The examples were presented and commented on by departmental anesthesiologists. The final institutional-specific examples are italized in the table. ASA-approved examples are not underlined. In step 3, this table was used in education of clinicians (anesthesiologists, residents, nurse anesthetists, nurses in the pre-anesthesia clinic) and posted in the pre-anesthesia clinic
