- This table displays the results of the responses to survey questions regarding the usability of the two activity questionnaires employed for patient self-triage of functional capacity, the Duke Activity Status Index (DASI), and the Patient-Reported Outcomes Measurement System (PROMIS)–Short Form 12a–Physical Function.
